1. ALL ENTRIES MUST BE TYPEWRITTEN OR LEGIBLY WRITTEN.
2. PRICE VALIDITY SHALL BE FOR A PERIOD OF (30) CALENDAR DAYS.
3. PHILGEPS, BIR COR, AND LATEST MAYOR'S/BUSINESS PERMIT SHALL BE SUBMITTED.
4. BIDDER SHALL SUBMIT OMNIBUS SWORN STATEMENT IF THE ABC OF THE PROCUREMENT PROJECT IS ABOVE PHP 50,000.00 AND FOR THOSE WITH SVP AS MODE OF PROCUREMENT.
5. BIDDER SHALL SUBMIT INCOME TAX RETURN FOR THE FOLLOWING PROCUREMENT PROJECT: (A.) SMALL VALUE PROCUREMENT (SVP) IF ABC IS ABOVE PHP 500,000.00
(B.) LEASE OF REAL PROPERTY OR VENUE (LRPV), AND (C.)SCIENTIFIC SCHOLARLY OR ARTISTIC WORK EXCLUSIVE TECHNOLOGY AND MEDIA SERVICES IF ABC IS ABOVE PHP 500,000.00
6. BIDDER SHALL SUBMIT BROCHURES SHOWING THE PRODUCT BEING OFFERED.
7. BID DOCUMENTS MUST BE SENT THROUGH SEALED ENVELOPE OR VIA EMAIL WITH PASSWORD PROTECTED ON OR BEFORE OPENING OF BID.
