  2. PhilGEPS Certificate of Registration (if applicable) or PhilGEPS Registration number;
  
  3. Omnibus Sworn Statement- Duly Signed and Notarized (can be downloaded at the GPPB Website); and
  
  4. For:
  
  a.) Corporations - Corporate Secretary's Certificate delegating authority to transact on behalf of the service provider with the supplier's Securities and Exchange Commission (SEC) Certificate

  b.) Sole Proprietorships - DTI Certificate and Special Power of Attorney (SPA) if applicable...
