A) REQUEST FOR QUOTATION

1. One (1) Lot Gift Certificates in the following denominations:
1000.00 x 796 pcs =
500.00 x 236 pcs =
100.00 x 524 pcs =

Terms and Conditions:

1. May be used to purchase merchandise or goods.
2. Acceptable and redeemable:
a. in at least two (2) department store chains; or
b. in a department store and grocery/supermarket
(Department Store must have location in Metro Manila and at accredited retail/establishments nationwide)
3. No expiration of validity of the certificate.
4. List of merchants that will honor the gift certificates are accessible online or through a brochure.
5. The gift certificate should be in denominations of P 1000.00, P 500.00, P 100.00.

OP-s Required Delivery Period (DP): Seven (7) working days upon receipt of Contract
