Prices shall be quoted in Philippine Peso and the Supplier shall submit the duly signed and accomplished Request for Proposal in a sealed envelope address to the Bids and Awards Committee (BAC) or the Procurement BAC Secretariat of the DAVAO CENTER FOR HEALTH DEVELOPMENT at J.P. Laurel Avenue, Bajada, Davao City.
