Request for Proposal (RFP) Forms may be obtained from the Procurement BAC Secretariat, Davao Center for Health Development, J.P. Laurel Avenue, Bajada, Davao City, from 8:00 A.M. to 5:00 P.M., or downloaded from:

Prices shall be quoted in Philippine pesos. The Supplier shall submit the duly signed and accomplished RFP in a sealed envelope addressed to the Bids and Awards Committee (BAC) or the Procurement BAC Secretariat at the above office address.

Alternatively, the accomplished RFP Form may also be submitted via email to davaochdprocurement@gmail.com

The Davao Center for Health Development reserves the right to accept or reject any proposal, annul the procurement process, or reject all proposals prior to contract award without incurring any liability to the affected suppliers.
