Description : School forms are considered official documents. it contains the students- grades and teacher`s reports that are essential. that is why writing it clearly, legibly, and accurately is a must and to avoid errors of the data. Official DepEd School Forms are vital in every teacher's daily routine. Reports and forms are the decorations in the teaching profession. Teachers are bound to submit their reports all throughout the school year. There are various Official DepEd School Forms and each one of them has a sole purpose. Like the ECCD (Early Childhood Care and Development) ASSESSMENT which has a goal to comprehensively understand a child's development and readiness in various domains, such as cognitive, social-emotional, language, and motor skills. Our school needs to reproduce hard copies of the said assessment for the SY 2025-2026.

The procurement and implementation of this proposal shall be chargeable to its MOOE funds, as provided for under Paragraph 4, Section 5.D of DepEd Order No. 008, s. 2019 entitled “Revised Implementing Guidelines on the Direct Release, Use, Monitoring and Reporting of Maintenance and Other Operating Expenses”.b...
