  Outside Labor, Equipment & Materials for the Replacement
  of Tympanometer Probe for One (1) unit of Tympanometer,
  with SN. 9300804 at Newborn Screening with the following
  Scope of Works including required parts:
  I. Replacement of 2 pcs. Tympanometer Probe (Brand New)
  II. Cleaning of the Equipment.
  III. Testing and Commissioning including Calibration.
  
  Note: Cost Inclusive of Labor Cost and other charges.
  
  Conditions/Requirements:
  1. Supplier must secure approved Work Permit & submit complete list of
  names of technicians at EFMS Office prior to repair activities.
  2. Submit Service Report/Certification of Equipment Operation and
  Calibration Report upon completion of the repair, validated and signed
  by the EFMS Representive.
