  Outside Labor, Equipment & Materials for the
  Repair of Three (3) units Novos Incubator KI-1000,
  SN. KI-9899-538, KI-9899-540, KI-9899-541
  at Neonatal ICU with the following scope of works
  
  I. Replacement of:
  a) 6 pcs Front Door, Canopy, Parts #001 796 (2 pcs each per unit)
  II. Cleaning of the Equipment
  III. Testing and Commissioning
  
  Note: Cost Inclusive of Labor Cost and other charges
  
  Conditions/Requirements:
  1. Supplier must secure approved Work Permit & submit complete list of
  names of technicians at EFMS Office prior to repair activities
  2. Submit Service Report/Certification of Equipment Operation & Calibration
  Cert. upon completion of the repair, validated and signed by the EFMS
  Representative and by the End-user.
  Delivery Period: One (1) Month upon receipt of P.O.
  Warranty: One (1) Month
