Description : The Isabela State University - Cauayan Campus invites registered contractors/ suppliers/bidders to submit bid proposals/price quotation of the items listed in the attached RFQ/RFP.

Upon submission of duly accomplished and signed Request for Quotation Form (RFQ) and Bill of Quantities (BOQ), the following documents shall be attached. Failure to comply/submit with any of the following requirements shall be a basis for disqualification

a. Valid Business/Mayor-s Permit;
b. Valid PhilGEPS Registration Certificate ( for Platinum) or PhilGEPS Number (for red only)
c. Valid DTI Registration/SEC Certificate of Incorporation
d. Latest Income/Business Tax Return (stamped "received" by the BIR);
e. Omnibus Sworn Statement (to be submitted by the winning bidder before the issuance of Purchase Order/Job Order)
f. Valid Tax Clearance Certificate
g. Valid PCAB License
h. Site Inspection Certificate (Contact the Procurement Office regarding the schedule of site inspection. Non-compliance with the site inspection requirement and schedule shall be a ground for disqualification)
