Please quote the price on the item(s) listed below, subject to the Terms and Conditions stated below and submit in a sealed envelope your quotation duly signed by your authorized representative not later than 12:00 N.N of AUGUST 29, 2025, together with the requirements mentioned in No. 5.

TERMS AND CONDITIONS:
1. All entries must be typewritten or legibly written.
2. Delivery period is within Sixty (60) Calendar Days or upon receipt of the approved funded Purchase Order (P.O.). Administrative penalties pursuant to Sec. 69 of the Revised IRR-RA 9184, as amended, shall be imposed for non-delivery without valid reason.
3. Warranty requirement shall be for a minimum of three (3) months for supplies & materials; one (1) year for Equipment; 3 years for IT Equipment from date of acceptance by the end-user.
4. Price validity shall be for a period of FIVE (5) MONTHS.
5. Submit updated PhilGEPS Registration Certificate; latest Mayor's Permit, Tax Clearance; and/or Income/Business Tax Return (For ABCs above Php500k); and/or Omnibus Sworn Statement (For ABCs above Php50k)...
