GENERAL CONDITIONS:
1. The quotation may be typewritten or handwritten using this form. You may also use your own form, provided that it is duly signed by an authorized representative. Unsigned quotations shall be automatically rejected.
2. The submission period of the quotation is until December 26, 2025 with delivery period/terms of thirty to forty-five (30-45) days.
3. Prices quoted shall be VAT inclusive and shall remain valid for sixty (60) calendar days.
4. PhilGEPS Registration Number/ Certificate, Mayor-s/ Business Permit, NGPA Omnibus Sworn Statement (for ABCs above P200K) and Annual Income Tax Return (for PRs/ JORs with PhP 500K ABC) shall be attached upon submission of the quotation.
