    b) COMPANY NAME;
    c) REQUESTING UNIT; and
    d) DEADLINE OF SUBMISSION

    (2) COMPUTATION OF UNIT PRICES, TOTAL UNIT PRICES & TOTAL AMOUNT (GRAND TOTAL) SHALL BE PROPERLY FILLED OUT. YOU MUST COMPUTE ALL THE NECESSARY PRICES IN ORDER TO FACILITATE THE OPENING OF THE SEALED CANVASSES (REQUEST FOR QUOTATION) AND TO AVOID MISCALCULATIONS. IF YOU HAVE NO BID ON THE SPECIFIC ITEM, KINDLY INDICATE ZERO (0).
