REQUEST FOR QUOTATION (RFQ)

The Local Government Unit of La Paz, Leyte, through its Bids and Awards Committee (BAC), will undertake a Negotiated Procurement - Small Value Procurement (Sec.53.9 IRR RA 9184) under Purchase Request No. 25-0042, 25-0043, 25-0044 with an Approved Budget for the Contract amounting to One Hundred Twenty-Five Thousand Pesos (Php 125,000.00) only.

In accordance with the above provision the canvass forms can be obtained from the BAC Secretariat, G/F La Paz Town Hall, LGU - La Paz, Leyte, starting May 20, 2025, from 8:00 A.M. to 5:00 P.M. Deadline for the submission of canvass is on May 23, 2025, 5:00 P.M. at the same office along with the following requirements for accreditation purposes:

1.Valid and Current Mayor-s Permit/Municipal License;
2.PhilGEPS Certificate of Registration;
3.Omnibus Sworn Statement
