3. The following requirements must be submitted together with the quotation: Mayor-s/ Business Permit. (In lieu of Mayor-s Permit and Philgeps Registration Number, Certificate of Platinum Membership may be submitted)
4. Omnibus Sworn Statement shall be required to the Lowest Calculated and Responsive Quotation before the issuance of Purchase Order/ Contract (For Small Value Procurement with ABCs above P50K and Emergency Cases with ABCs above P500K)
