PURCHASE OF OFFICE FURNITURES

The Civil Aviation Authority of the Philippines (CAAP) intends to apply the estimated amount of Fifty-two Thousand Pesos (P 52,000.00) for the Purchase of Office Furnitures project.

The CAAP now requests you to submit price quotation on the item listed below, subject to the Specification and Terms and Conditions provided hereunder.

1. All entries must be printed.
2. Number of days to complete: Fourteen (14) calendar days.
3. Bid must be complete and award will be made be on a lump sum basis.
4. Price validity shall be for a period of thirty (30) calendar days.
5. The following should be submitted together with the quotation:

5.1 Phil-GEPS Registration Certificate/Number;
5.2 Current Business/Mayor-s Permit;
5.3 DTI/SEC Registration Certificate;
5.4 Tax Clearance Certificate per EO No. 398

6. Other documentary requirements:

6.1 Omnibus Sworn Statement; and

6.2 Latest Income & Business Tax Returns;
