1. Secure Request for Schedule of Delivery at the DOH Supply Office Prior to delivery.
2. Provide the Health Promotion Unit a copy of the request.
3. Notify the Health Promotion Unit upon delivery of materials
4. Provide the Supply Section the Statement of Account upon full delivery.

Schedule of Delivery: March 10, 2025 or 15-30 days upon receipt of the approved P.O.
