Description : TERMS AND CONDITIONS

All entries must be typewritten or legibly written.

Delivery period shall be within thirty (30) calendar days upon receipt of the approved funded Purchase Order (P.O.). Administrative penalties pursuant to Section 99 of the IRR of RA 12009 shall be imposed for non-delivery without valid reason.

Warranty shall be for a minimum of three (3) months for supplies and materials, and one (1) year for equipment, from the date of acceptance by the end-user.

Price validity shall be for a period of sixty (60) calendar days.

For suppliers, the two (2)-envelope system shall be followed.

First Envelope - must contain and shall be submitted together with the quotation the following documents:
a. PhilGEPS Platinum Certificate (certified copy) or PhilGEPS Registration Number
b. Mayor-s/Business Permit
c. Latest Income Tax Return (ITR)

Second Envelope - shall contain the Request for Quotation (RFQ) and will only be opened after the first envelope is declared complying.
