The Local Government Unit of Balbalan through its Bids and Awards Committee (BAC) intends to procure CPR MANIKINS with an Approved Budget for the Contract (ABC) of ONE HUNDRED FIFTY THOUSAND PESOS (Php 150,000.00) through Section 34 Small Value Procurement of the Implementing Rules and Regulations of Republic Act No. 12009.

Interested bidder shall submit a copy of the following documents along with the quotation on or before the specified deadline of submission of quotation:

a. Mayor-s/Business Permit
b. PhilGEPS Registration Number
c. Notarized Omnibus Sworn Statement and if applicable, Original Notarized Secretary-s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney of all members of the Joint Venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder.
