Description : TERMS AND CONDITIONS:
1. The supplier shall inform in writing the procuring entity in advance prior to the delivery schedule at least five calendar (5) days from the actual delivery.
2. Schedule of delivery is from Monday to Friday from 8:00am to 5:00pm except holidays
3. All deliveries shall be accompanied by an authorized representative.
4. Items to be deliverd shall have a shelf life of 18 months and above expiration date upon receipt by the procuring entity (Materials and Management Unit). A duly approved guarantee letter shall be required in the event that the item to be delivered is below 18 months.
5. Certificate of Product Registration of the items bid shall be attached. Nearly expired CPR submitted during bidding shall provide an updated CPR upon delivery.
