Description : Terms of Reference:
1. Expiration date should not be less than two (2) years from the date of manufacture and not less than 18 months from the date of delivery. For items with shorter expiration date, the Supplier shall submit a Guarantee Letter prior to delivery for the approval of the Head of the Procuring Entity (HOPE) of his/her autohired representative. Unutilized medical supplies shall be replaced within three (3) months prior to the date of expiration and replaced supplies shall delivered within fifteen (15) calendar days upon receipt of the said items. Otherwise, the corresponding amount shall be deducted from any amount due to the Supplier.
2. Supplier shall inform in writing the procuring entity in advance prior the delivery schedule at least five (5) days from the actual delivery.
3. Delivery period is 15 calendar days from receipt of Notice to Proceed.
