Description : TERMS AND CONDITION:
1. The supplier shall inform in writing the procuring entity in advance prior to the delivery schedule at least five calendar (5) days from the actual delivery.
2. Schedule of delivery is from Monday to Friday from 8:00 a.m. to 5:00 p.m. except holidays.
3. All deliveries shall be accopanied by an authorized representative.
4. Items to be delivered shall have a shelf life of 18 months and above expiration date upon receipt by the procuring entity (Materials and Management Unit). A duly approved guarantee letter shall be required in the event that the item to be delivered is below 18 months.
5. Certificate of Product Registration of the items bid shall be attached. Nearly expiry CPR submitted during bidding shall provide updated CPR upon delivery.
6. Delivery shall be on quarterly basis or as the need arises. First delivery (3 boxes of 25s) succeeding delivery shall be every first week of the quarter.
