REQUEST FOR QUOTATION (RFQ)

The Local Government Unit of La Paz, Leyte, through its Bids and Awards Committee (BAC), will undertake a Negotiated Procurement - Small Value Procurement (Sec.53.9 IRR RA 9184) under Purchase Request No. 25-0069 with an Approved Budget for the Contract amounting to Eighty Thousand Eight Hundred Fifty Pesos (Php 80,850.00) only.

In accordance with the above provision the canvass forms can be obtained from the BAC Secretariat, G/F La Paz Town Hall, LGU - La Paz, Leyte, starting March 7, 2025, from 8:00 A.M. to 5:00 P.M. Deadline for the submission of canvass is on March 10, 2025, 5:00 P.M. at the same office along with the following requirements for accreditation purposes:

1.Valid and Current Mayor-s Permit/Municipal License;
2.PhilGEPS Certificate of Registration;
3.Omnibus Sworn Statement
