Description : Provision of On-site Mandatory Drug Testing for OCD Central Office Employees
Scope of Work:
• Conduct On-site drug testing for 465 personnel
• The service provider must be capable of completing the on-site testing within four (4) days.
Service Provider Requirements:
• Must provide one (1) drug test kit per individual (465 pcs).
• Must supply all necessary medical supplies and equipment to conduct the test for 465 pax.
• Must deploy licensed and qualified medical professionals (registered medical technologist, nurses).
• Must shoulder all expenses related to the conduct of the testing including meals of the staff.
• Must cover all costs for evaluation of results, including free confirmatory testing for initially positive specimens.
• Must submit test results to OCD within fifteen (15) calendar days after testing.
• Must ensure confidentiality and data privacy in accordance with RA 10173 (Data Privacy Act of 2012)
• Must submit a Certificate of Good Standing from DOH
