Provision of Office Supplies
1. The Department of Health, Batanes General Hospital (Procurement under Alternative Method of Procurement - Shopping) invites local traders/dealers/distributors who can submit the best offer for the Provision of Office Supplies.
The Approved Budget for the Contract (ABC) to payments under contract for the Whole Lot is Php 13,109.00.
2. Source of Fund: Internally Generated Fund
3. Delivery period shall be within 30 calendar days upon receipt of Purchase Order.
4. For evaluation purposes, prospective bidders are required to submit the following documents:
a) Valid Mayor-s Permit issued by the city or municipality where the principal place of business of the prospective bidder is located;
b) Duly accomplished Request for Quotation Form.
c) PhilGeps certificate of registration.
