3. Omnibus Sworn Statement*

*The Omnibus Sworn Statement may be submitted together with the RFQ Form or prior to the issuance of award.

Submission of Quotation will be on or before 2:00PM of June 16, 2025.
Opening of Quotations will be held on PENRO Conference Room, 2:30 PM on June 16, 2025.
