Interested Service Provider shall submit their filled-up Request for Quotation (RFQ) to this Office (walk-in or email)
together with the updated copy of the following requirements:

1. Mayor's/Business Permit; and
2. PhilGEPS Registration Number/Certificate

The Omnibus Sworn Statement may be submitted together with the quotation or prior to the issuance of award.

Submission of Quotation will be on or before 1:00PM of February 11, 2025.
Opening of Quotations will be held on PENRO Conference Room, 2:00 PM of February 11, 2025.
