A. Health Care Packages

2. In-Patient Care
3. Out Patient Care
4. Emergency Care
5. Dental Care

• Pre-Existing Conditions (All Pre-existing Conditions shall be deemed covered
• Semi-Private/Private Room and with a maximum benefit of at least 75,000.00 per illness and restored after 45 days and with access to major hospitals in Caraga

B. HMO Qualifications:
1. Reputable HMO/Health Insurance Company
2. Preferably with Regional Office/Satellite Office within Caraga (50km radius from Bayugan City)
3. Number of accredited Hospitals within Caraga (with attached List)
4. Number of DOH and PhilHealth Accredited Hospitals
5. Number of Accredited Doctors in Caraga Region Hospitals
