100 pcs Production and Printing of Advocacy Polo Shirts in connection to the Conduct of DOH Information Systems Strategic Plan (ISSP) 2028-2030 Planning Workshop - Batch 4 in Boracay, Malay, Aklan to wit;

Delivery Procedure:
1. Secure request for schedule of delivery at the DOH Supply Office prior to delivery.
2. Provide the ICT Unit a copy of the request.
3. Notify the ICT Unit upon delivery of materials.
4. Provide the Supply Section the Statement of Account upon full delivery

Prerequisites for the Production Process:
1. Supplier must be available anytime to collaborate with the
end user during the layout process.
2. All materials must be approved by the end-user prior to production.
3. Sample copy of the final version of materials must be
submitted to the end user prior to production.
