1) Mayor's Permit / Business Permit
2) Philgeps Certificate
3) Income/Business Tax Return (Latest Quarter)
4) Company Profile
5) Quoted price should be inclusive of shipment and/or handling fee from the place of goods' origin to Sulu Sanitarium and General Hospital.
6) Delivery must be within 7 calendar days upon receipt of the Approved Purchase Order.
7) Expiry date must not be less than 36 months.
8) Bidders must submit Certificate of Product Registration / Sample Photo with the detailed description of the design and materials used for the manufacturing or composed.
