2. PhilGEPS Registration Number/Certificate; and
3. Omnibus Sworn Statement*

*The Omnibus Sworn Statement may be submitted together with the RFQ Form or prior to the issuance of award.

Submission of Quotation will be on or before 5:00PM of August 26, 2025.
Opening of Quotations will be held on PENRO Conference Room, 11:00 PM on August 27, 2025.
