NOTE: 1. All entries must be filled.
2. Delivery period shall be within Seven (7) calendar days from receipt of the P.O / Contract.
3. Warrant shall be for a period of six (6) months for supplies and materials and one (1) year for equipment,
for the date of acceptance by the procuring entity.
4. Price validity shall be for a period of thirty (30) calendar days.
5. G-EPS Registration Certificate and other bid documents shall be attached upon submission of the
quotation.
