Terms and Condition:
1. Schedule of annual medical & physical examination shall be conducted any
day on the employee-s birth month.
2. CAAP-CBO shall designate and establish agreement with one (1) DOH
Licensed/Accredited Clinic or Health Facility that can provide the required
Diagnostic and screening procedures/examinations of the CAMPEx.
