Description : Annual Physical Examination Services for the 17 Employees of SSS Kidapawan branch with the following conditions:
I. Required Laboratory Tests
a. Urinalysis
b. Complete Blood Count
c. Chest X-Ray
d. Electrocardiogram
e. Blood Chemistry Panel: FBS, SGPT, BUN, CREA, BUA, Lipid Profile
II. Other test as required y branch medical specialist
-HBA1C, Papsmear, Breast Mammogram, Whole Abdomen Ultrasound, Prostate specific antigen, TSH, FT4 (Selected Employees Only)
