PROJECT: SUPPLY AND DELIVERY OF 300 PCS TWO TONED POLO SHIRT WITH EMBROIDERY

LOCATION: PCA CENTRAL OFFICE R&D BLDG., DILIMAN, QUEZON CITY

I. SCOPE OF WORK

Supply and delivery of 300 pcs Two toned Polo Shirt with embroidery with the following specifications:

1. 300 PCS TWO-TONED POLO SHIRT WITH EMBROIDERY

• Sizes: XS, S, M, L, XL, 2XL, 3XL
• Material: Cotton Honeycomb (100% Cotton, 0% Polyester)
• Base Color: Olive green with avocado green and white combination
• Collar and Right Sleeve: Olive green and avocado green line
• Cuffs: Avocado green
• Front Design: Embroidered of PCA logo on left side
• Back Design: Embroidered 52nd Anniversary on upper back part (See attached design)

2. TOTAL ABC: One Hundred Ninety Five Pesos Only. (₱ 195,000.00)
