Please quote your lowest price inclusive of VAT for the item(s) listed below, subject to the General Conditions stated on this quotation that must be duly signed by your authorized representative and shall be submitted through an enclosed envelope not later than three (3) working days upon receipt of this request.

Note:
1. ALL ENTRIES MUST BE TYPEWRITTEN.
2. DELIVERY PERIOD WITHIN SEVEN (7) CALENDAR DAYS.
3. WARRANTY SHALL BE SIX (6) MONTHS FOR SUPPLIES AND MATERIALS AND ONE (1) YEAR FOR EQUIPMENT FROM THE DATE OF ACCEPTANCE BY THE PROCURING ENTITY.
4. PRICE VALIDITY SHALL BE FOR THIRTY (30) CALENDAR DAYS.
5. PhilGEPS REGISTRATION CERTIFICATE SHALL BE ATTACHED UPON SUBMISSION OF THE QUOTATION.
6. BIDDERS SHALL SUBMIT AN ORIGINAL BROCHURE SHOWING CERTIFICATIONS OF THE PRODUCT BEING QUOTED.
