REQUEST FOR QUOTATION

PROCUREMENT OF MULTI-PURPOSE PAPER
SHOPPING-25-001

1. The Tondo Medical Center (TMC), hereinafter referred to as “the Purchaser”, has a requirement for the Procurement of Multi-Purpose Paper with an Approved Budget for the Contract (ABC) of Four Hundred Eighty-Two Thousand Pesos (Php 482,000.00).
2. Bidding procedures will be conducted in accordance with the Revised Implementing Rules and Regulations of Republic Act 9184 (R.A. 9184).

3. Quotations must be delivered at the PROCUREMENT SECTION/Tondo Medical Center, Balut, Tondo, Manila or send email at procurement@tmc.doh.gov.ph on or before 16 January 2025, 9:00AM.
4. Your price quotation must be quoted in Philippine Peso and must include the unit price and total price, inclusive of all taxes to be paid if the contract is awarded.

5. Bids shall be valid for ninety (90) calendar days from the Opening of Bids.

6. The Delivery period is AS CALLED FOR upon receipt of approved Notice to Proceed (NTP).
