Description : 1. Bidders shall provide correct and accurate information required in this form.
2. The Project shall be awarded as One Project having several items that shall be awarded as one contract. Quotation received exceeding each total cost per item and/ or the total Approved Budget for the Contract shall be automatically rejected.
3. Price quotation/s shall be denominated in Philippine Peso and shall include all taxes, duties and/or levies payable.
4. Warranty shall be for a period of Six (6) months for Supplies and Materials. One (1) year for Equipment, from the date of acceptance by the Procuring Entity.
5. Price quotation/s shall be valid for a period of thirty (30) calendar days from the date of submission.
6. Award of contract shall be made to the lowest quotation which complies with the technical specifications and other terms and conditions stated herein.
7. Any interlineations, erasures, or overwriting shall be valid only if they are signed or initiated by you or any of your duly authorized representative/s.
