1. BIDDERS SHALL PROVIDE CORRECT AND ACCURATE INFORMATION REQUIRED IN THIS FORM.
2. DELIVERY PERIOD WITHIN 15 CALENDAR DAYS FROM PERFECTION OF CONTRACT OR PURCHASE ORDER.
"3. WARRANTY SHALL BE FOR A PERIOD OF NOT LESS THAN SIX (6) MONTHS FOR SUPPLIES AND MATERIALS AND NOT LESS THAN

7. ALTERATION OF THE ITEM AND DESCRIPTION SHALL BE DISQUALIFIED.
8. THE ITEM(s) SHALL BE DELIVERED ACCORDING TO THE REQUIREMENTS SPECIFIED IN THE TECHNICAL SPECIFICATIONS.
