Interested Suppliers shall submit their filled-up Request for Quotation (RFQ) to this Office (walk-in or email)
together with the updated copy of the following requirements:

1. Mayor's/Business Permit;
2. PhilGEPS Registration Number/Certificate; and
3. Omnibus Sworn Statement*

*The Omnibus Sworn Statement may be submitted together with the RFQ Form or prior to the issuance of award.

Submission of Quotation will be on or before 5:00PM of September 22, 2025.
Opening of Quotations will be held on PENRO Conference Room, 11:00 AM on September 23, 2025.
