Description : Please quote your lowest price inclusive of VAT on the item/s listed below, subject to the Terms and Conditions of this RFQ, and submit your quotation duly signed by your representative not later than December 26, 2025 at Bids and Awards Committee LGU-Capoocan, Leyte.

Upon receipt of at least one (1) quotation on or before the submission deadline, the BAC shall open the quotations.

Sealed quotations and other requirements stated below shall be submitted to the Bids and Awards Committee (BAC) at Municipal Hall, LGU-Capoocan, Leyte.

Supplier shall submit the following requirements in certified photocopy from the original.

a. Duly signed Request for Quotation. Prices shall be quoted in Philippine Peso. Statement of Compliance must be accomplished by supplier.
b. PhilGEPS Registration Certificate
c. Valid Mayor-s Permit,
d. Original brochures of the items offered showing its performance characteristics or certifications, if applicable
