  Certificate of Registration;
  
  2. Valid and Latest Business/Mayor-s Permit issued by the city or municipality where the
  principal place of business of the bidder is located, or a Recently Expired Business/Mayor-s
  Permit for CY2025 and the Official Receipt as proof of Renewal for CY2026; and
  
  3. Original and Notarized Omnibus Sworn Statement
