NOTE TO SUPPLIER: Conduct of APE should be at SSS Baguio Branch; supplier should have a mobile x-ray laboratory. Except for Pap Smear and Breast Mammogram - procedure may be undertaken at supplier's own laboratory center which should be located within Baguio City.
