1. ONE YEAR PROVISION OF JANITORIAL SERVICES AT BONBON NHS RENDERING ONE SHIFT:
- SIX DAYS A WEEK (INCLUDING HOLIDAYS)
- 8 HOURS

2. THE SERVICE PROVIDER SHALL PROVIDE ONE UTILITY PERSONNEL WHO IS:
A. MALE, PREFERABLY LIVES IN BONBON, CDOC:
B. OF GOOD MORAL CHARACTER AND WITHOUT CRIMINAL OR POLICE RECORDS:
C. PHYSICALLY AND MENTALLY FIT, AS EVIDENCED BY THE MEDICAL CERTIFICATE:
D. DULY TRAINED AND SKILLED TO FUNCTION AS UTILITY PERSONNEL:
E. MUST KNOW HOW TO DO MINOR MAINTENANCE REPAIRS, ELECTRICAL, MASONRY AND PLUMBING IN THE SCHOOL
F. FULLY VACINNATED

3. THE SERVICE PROVIDER AND UTILITY WORKERS SHALL PERFORM DUTIES AND RESPONSIBILITIES IN ACCORDANCE WITH THE TERMS OF REFERENCES FOR THE CONTRACT.

4. MUST HAVE SSS, PAG-IBIG, PHILHEALTH NUMBER OR ACCOUNT WITH UPDATED MONTHLY CONTRIBUTION REMITTANCES.
