Interested Service Providers shall submit their filled-up Request for Quotation (RFQ) to this Office (walk-in or email)
together with the updated copy of the following requirements:

1. Mayor's/Business Permit;
2. PhilGEPS Registration Number/Certificate;
3. Business/Income Tax Return; and
4. Omnibus Sworn Statement*

*The Omnibus Sworn Statement may be submitted together with the RFQ Form or prior to the issuance of award.

Submission of Quotation will be on or before 1:00PM of November 18, 2025.
Opening of Quotations will be held on PENRO Conference Room, 3:00 PM on November 18, 2025.
