GENERAL
• HMO provider must comply with the requirements under the provisions of Department of Budget and Management (DBM) Budget Circular No. 2024-6, such as but not limited to Insurance Commission registration.
• With at least four (4) accredited hospitals, clinics or diagnostic centers in each of the provinces of Region I (La Union, Pangasinan, Ilocos Sur, and Ilocos Norte)
• Coverage period of at least one (1) year
• Principal enrollees must be between 18 and 65 years of age
• Maximum benefit of at least Fifty Thousand Pesos (P50,000.00) per year based on the schedule of disability
• Annual mode of payment

OUTPATIENT EMERGENCY CARE AND HOSPITALIZATION, inclusive of at least the following:
• Room and board
• Doctor-s fees
• Laboratory and diagnostics procedures
• Special modalities of treatment as medically necessary during ER
• Medicines as medically necessary during ER and confinement, subject to standard inner limits
• Diagnostics and therapeutic procedures as medically necessary during ER and confinement.
