Description : • HMO provider must comply with the requirements under the provisions of Department of Budget and Management (DBM) Budget Circular No. 2024-6, such as but not limited to Insurance Commission registration.
• With accredited hospitals in the provinces of Region I (La Union, Pangasinan, Ilocos Sur, and Ilocos Norte)
• Coverage period of at least one (1) year
• Principal enrollees must be between 18 and 65 years of age
• Maximum benefit of at least Seventy Thousand Pesos (P70,000.00) per illness per member every year
• Annual mode of payment

1. PREVENTIVE HEALTHCARE
Annual Physical Examination (APE), inclusive of at least the following:
a. Complete Blood Count
b. Urinalysis
c. Fecalysis
d. Chest X-ray
e. Electrocardiogram
f. Pap Smear

2. IN-PATIENT CARE, inclusive of at least the following:
a. Semi-Private Room
b. Operating room and Recovery room
c. Administered medicines
d. ICU confinements
e. X-ray and laboratory examinations

3. OUT-PATIENT CARE, inclusive of at least the following:
a. Referral to specialists
