Description : Procurement of Health Maintenance Organization (HMO) Product - Health Care Services for 1,100 enrolled members. The HMO shall cover the following benefits for a period of 12 months;

Maximum benefit limit of ₱ 80,000.00 per year.
a. In-patient benefit of ₱ 60,000.00 per confinement
b.1. Out-patient and ER benefit such as consultation or teleconsultation, diagnostic procedure and laboratory test.
b.2. Annual Physical Exam and five (5) laboratory test including X-ray
c. Dental Benefit - extraction, consultation, fillings and re-cementation, gum treatment

Conditions:
1. HMO provider must have:
a. Accredited/affiliated hospital in Dumaguete City and Cebu City
b. Accredited/affiliated Dental Clinic in Siquijor Province
c. Accredited/affiliated Medical Clinic and Laboratory in Siquijor Province
