Description : Procurement of Health Maintenance Organization (HMO) Product - Health Care Package for Sixty (60) Enrolled Members

Maximum benefit limit of 80,000.00 per year.
a. In-patient benefit of 40,000.00 per confinement
b. Out-patient and ER benefit such as consultation or teleconsultation, diagnostic procedure and laboratory test.
c. Annual Physical Exam and five (5) laboratory test including X-ray
d. Dental Benefit - such as consultation, fillings, extraction, and gum treatment

Terms and Conditions:
1. The HMO provider must have accredited/affiliated doctors, hospitals, clinics, and laboratories in Dumaguete City and Cebu City.
2. The HMO package shall be available within 15 days after the receipt of the Purchase Order.
3. The HMO product coverage shall be for a period of 12 months.
4. The HMO membership card shall be delivered at the office of SDO, Siquijor, Helen, Larena, Siquijor. The deliveries shall be completed within 15 days after the receipt...
