I. To implement the provisions of EO No. 64, s. 2024, DBM Circular No. 2024-6, and DepEd Order No. 016, s. 2025 by ensuring that all eligible personnel benefit from the mandated medical subsidy.

II. COVERAGE OF THE PROPOSED HMO PLAN (GROUP AVAILMENT)
The HMO plan to be procured shall include the following services:

• Fecalysis (Stool Exam)

• Electrodiagram (covered for adults age 40 and above, or if prescribed)

• Operating Room and Recovery Room up to Maximum Benefit Limit
• Administered medicines up to Maximum Benefit Limit
