4. MOA of Transporter and Treater with DENR-EMB Registered Final Disposal Facility
5. MOA of Transporter and Generator Facility
6. Updated Transporter Managing Head Certificate of Training from DENR-EMB accredited training organization
7. Updated Certificate of Training and accredited Pollution Control Officer
8. Latest Proof of treatment (with photos), transport manifest, Treatment Certificate, and Certificate of Final Disposal (within 1 year)
9. Proof of training and/or updated accreditation of waste handlers from DENR-EMB of their accredited training organizations.
